• You need to fill out a Change of Degree Status or Curriculum Form for the Graduate School.
• After you complete the form, sign it and email it to the program coordinator at operationsresearch@ncsu.edu.

• You need three people to submit Letters of Recommendation and forms directly to our program coordinator at operationsresearch@ncsu.edu. After that, submit a new Statement of Purpose to our Executive Assistant at the same email.
• Additionally, fill out a Change of Degree Status or Curriculum Form for the Graduate School. Complete the form, sign it, and send it to our Executive Assistant at operationsresearch@ncsu.edu.
• If you are an international student, you will also need to submit the OIS Change in Curriculum/Level/Co-Major Request Form. The OR Director must sign this form before sending it to the Graduate School. You will also need to send your OIS Form to OIS.

• You need to fill out a Change of Degree Status or Curriculum Form for the Graduate School. Complete the form, sign it, and send it to our program coordinator at operationsresearch@ncsu.edu.
• Along with this form, you must submit the offer letter for your internship.
• If you are an international student, you must also submit the OIS Change in Curriculum/Level/Co-Major Request Form. This form has to be signed by the OR Director and sent to our program coordinator at operationsresearch@ncsu.edu.

• You need to fill out a Change of Degree Status or Curriculum Form for the Graduate School. Complete the form, sign it, and send it to our program coordinator at operationsresearch@ncsu.edu.
• You need three people to submit Letters of Recommendation and forms directly to our program coordinator at operationsresearch@ncsu.edu. Once they do that, submit a new Statement of Purpose and the OR Supplemental Questions to our Executive Assistant at the same email.
• If you are an international student, you must also submit the OIS Change in Curriculum/Level/Co-Major Request Form. This form has to be signed by the OR Director and sent to our program coordinator at operationsresearch@ncsu.edu.

The Graduate Plan of Work guides you to complete your degree and serves as a contract with your program.

Email the program coordinator at operationsresearch@ncsu.edu with your name and student ID #. Use the subject line: Reset PoW. The Graduate Office will reset your PoW so you can make changes and resubmit it for approval.

Review the OIS guidelines and requirements. Complete and sign the RCL request form, then submit it to the the program coordinator at operationsresearch@ncsu.edu. Get your PoW approved before you submit your RCL request. Enroll only in the courses listed on your PoW for the current semester.

• Fill out a Minor Declaration Form and bring it to your minor adviser. They will approve and send it to Registration & Records.
• “Degree Audit” in MyPack will show the minor audit. Your transcript will designate it when they confer your degree.

• Ph.D. or MSOR students must declare a minor. Complete the Minor Plan portion of your Plan of Work. MOR students do not have a minor. Please see Ph.D., MSOR, and MOR Degree Requirements pages.
